            Submit a UCWCP Enrollment Application by email, fax or mail. You can complete a printed form or use the online version with an electronic signature.

            Provide your company’s most recent workers’ compensation premium/payroll audit that includes payrolls by WC class code for work performed in Minnesota. You may use our Payroll Reporting Form.

             You may enroll at any time, regardless of your insurance renewal date.
         
IMPLEMENTATION
        - 
           Upon receipt of a fully completed application, an acceptance letter with an implementation kit and specific instructions will be issued. 
        
- 
           A training session will be provided for your office and field staff that interacts with injured workers.  
- 
             Your enrollment date will always be on the first of any month. Processing takes 10 days.
         
- 
             Only claims occurring after the date of enrollment are covered by the Program. 
         
- 
             Enrollment notification will be sent to your insurance provider's underwriter and your insurance agent so that premium credits can be applied to your policy.
